Disability Verification Guidelines

To qualify for disability services and resources, students are required to provide current documentation (completed within the last 3 years) from the appropriate licensed diagnostician familiar with the history and functional limitations of the impairment (see specific guidelines below). Failure to provide all required information may prevent the student from receiving requested accommodations and/or services.

Attention Deficit/Hyperactivity Disorder (AD/HD)

Qualified Diagnostician: Psychologist, Psychiatrist, or Physician
Required Documentation:
  • Medical or professional report documenting the disability
  • Statement of functional/educational limitations
  • Recommended accommodations and services


Learning Disabilities

Qualified Diagnostician: Ed/Clinical Psychologist or Psychologist
Required Documentation:
  • Psychological report documenting the disability
  • A complete achievement battery, an aptitude assessment, and an assessment of information processing
  • Statement of functional/educational limitations
  • Recommended accommodations and services


Deaf/Hard-of-Hearing

Qualified Diagnostician: Audiologist or Otologist
Required Documentation:
  • Audiologist report including current audiogram
  • Statement of functional/educational limitations
  • Recommended accommodations and services


Blind/Low Vision

Qualified Diagnostician: Ophthalmologist, Optometrist
Required Documentation:
  • Current vision test documenting the disability
  • Statement of functional/educational limitations
  • Recommended accommodations and services


Physical/Medical/Other

Qualified Diagnostician: Physician or Medical Specialist
Required Documentation:
  • Medical or professional report documenting the disability
  • Statement of functional/educational limitations
  • Recommended accommodations and services


Psychiatric Disabilities

Qualified Diagnostician: Psychiatrist or Psychologist
Required Documentation:
  • Clear statement of illness, including the DSM-IV diagnosis, summary of present symptoms and a prognosis
  • Statement of functional/educational limitations
  • Recommended accommodations and services
