Driver History Record Release/Request Form (Authorized Driver Database)

Springfield College is committed to providing the safest and most efficient modes of transportation in support of its official business, academic, athletic, and student life needs.

Vehicle safety is the responsibility of every driver. Whether operating a golf cart, cargo van, rental car or passenger van; any person seeking to operate a College rented, leased, or owned vehicle is expected to follow the rules and procedures established by the Transportation Committee (see Transportation Policy and Guidelines), and to abide by all campus regulations as well as all federal, state and local laws.

Vehicles are available for use by students, faculty and staff in order to facilitate official business, student life, athletic and academic activities. Personal use of College vehicles is not allowed. Official use includes, but is not limited to, authorized student organization/club travel, community service transportation, intercollegiate athletics, academic class trips/projects and authorized departmental event transportation needs.

Note: Any person seeking to drive a college rented, leased, or owned vehicle must be an authorized driver.

An authorized driver is defined by the College as any person eligible to operate a College owned, rented, or leased vehicle. Authorized drivers must adhere to the SC Transportation Service Policy and possess all of the following requirements*: Must be at least 20 years of age possess a valid United States driver’s license and have possessed a driver’s license for a minimum of 3 consecutive years successful completion of the College’s Defensive Driving Safety Program (or its equivalent) maintain an approved driver history. * See Transportation Policy; Procedures (I. All persons seeking to be authorized to drive for the college need to file an online driver history record release/request form. All perspective drivers are responsible for providing the SC Department of Public of Public Safety with a copy of their driver history. Any fees acquired from obtaining a driver history are the responsibility of the applicant. Filing the online form grants the SC Department of Public Safety permission to “run” your history for you. They are able to provide this service free of charge for most states. The states which either do not return records within a timely manner (2 to 6 weeks) or do not respond at all to the college’s inquiries are: CT, HI, IL, NJ, MD, PA, and WA.

Regardless of what state your license is issued, all persons looking to drive a college rented, leased, or owned vehicle must file an online Driver History Record Release/Request Form. A link to this form is located at the bottom of this webpage.

If you utilize an online agency to obtain a copy of your driver history, make sure that you will receive your copy in a timely manner.

If your license is issued from CT or PA the SC Department of Public of Public Safety can “run” your history but it could take 2 to 6 weeks to receive the history. If your driver history is needed sooner, you can obtain your history from your state’s DMV or from an online agency. WWW.DMV.org is such a site. The state DMV’s and online sites may require a fee.

If your license is issued from HI, IL, NJ, MD or WA you may have to obtain a copy of your history yourself. You can obtain your history from your state’s DMV or from an online agency. WWW.DMV.org is such a site. The state DMV’s and online sites may require a fee.

If you’re already an authorized driver and you’re seeking to maintain your authorization to drive status; you must provide the Department of Public Safety with an accurate driving history prior to its expiration. This process begins on an annual basis every August 1st

If you are not an authorized driver you need to file online driver history record release/request form and attend the College’s Defensive Driving Safety Program. Please try and file the form weeks prior to attending the course.

Student and Volunteer driver histories expire annually on October 31st. This means returning students are required to complete the driver history query annually (Anytime between August 1st 2008 and October 31st 2008). Student and volunteer driver histories are kept on file at the SC Department of Public Safety for one semester year.

Faculty and Staff driver histories will expire on January 1, 2009. This means in order to maintain authorization to drive a college rented, leased or owned vehicle, in the fall of 2008 returning faculty and staff will need to complete the driver history record release/request form. Please do this between August 1st 2008 and December 31st 2008 (before authorization to drive expires).

If you have problems with your web e-mail username and password, or if you get an error message that says "user not authenticated," please follows one of the directions as follows:

-If you are a Springfield College faculty/staff member, please contact the IT dept by filing a VHD request. You will not be able to proceed with your request until you have resolved any computer/user id/password problems you may have.

-If you are a Springfield College student, and you don't know/or you have trouble with your password or web login name, visit Room 021 in Babson Library with your valid SC student ID.

-If you are neither a Springfield College faculty/staff member nor a student, you must obtain a temporary volunteer web login name and password. To obtain the web login name and password, please contact Transportation Coordinator via E-mail: transportation@spfldcol.edu
